Home Hospice Community Mentoring Program (funded by the Cancer Council NSW and Social Ventures Australia) is FREE.
It is supports the carers of terminally ill people who want to live at home until the very end (over 80% of people want to die at home – only 20-30% are able to do so).
Community mentors, most with personal experience, provide information and guidance. This empowers carers – their circle of friends is brought together with a common purpose.
They are able to tap into local services and community resources, enabling the development of personal networks to share the load. This allows the carer to focus on enjoying the time they have left with their loved one, free from the burden of daily worries. It provides opportunities for family and friends to support each other, and to grant a very special last wish - the wish to die at home.
Eligibility:
Terminally ill people and/or their carers (all types of terminal illness)
