Home and Community Care (HACC) services
The HACC Program is a national program that provides basic, long-term services to people living in the community who are at risk of premature or inappropriate admission to long-term residential care. This may include frail older people and younger people with a disability, including children and young people of any age with moderate, severe or profound disabilities, and their carers.

In Northern Sydney there are many HACC funded services (flyers) across four sub-regional areas (map).
The type of services funded through the HACC Program include, but are not limited to:
| Domestic Assistance | Services may provide housekeeping, personal care, respite care or overnight care (for short periods only). |
| Social Support | Neighbour Aid - A range of support services such as shopping, linen service, individual transport and home visiting provided by volunteers. |
| Dementia Services | |
| Nursing Services | Home Nursing available 7 days a week between 8 am and 4.30 pm. An evening service is available to 10 pm. |
| Food Services | Delivery of hot and cold meals from Meals on Wheels provided 7 days a week. Centre based meals. Multicultural meal services may be available. |
| Respite Services | Care provided at home, at a day centre or on outings/recreation. |
| Day Centres | Care provided at home, at a day centre or on outings/recreation. |
| Home Modification | Installation, alterations and essential repairs to homes (i.e. safety ramps, rails, doorways, handheld showers & minor maintenance) |
| Gardening/Lawn Mowing | Conversion of gardens to low maintenance and provision of low cost lawn mowing services |
| Transport | Individual and Group transport including shopping and social outings in modified vehicles |
| Carer Support | Individual support, referral information and training for carers. |
| Community Options | Assessment of needs and co-ordination of services for people with high support needs. |

| Aged Care Assessment Team | Provides assessment, information and support for older people including assessment for aged residential care |
| Planning, Community Development & Coordination | Information, support, research, for the needs of older people and people with disabilities. Training for HACC funded services. |
